No matter what you do when you set out on the safety journey, you will never know what you don't know. You can give someone a title, a checklist and a system, but if they don't understand what they're looking at and don't feel the weight of it because no one has helped them see beneath the surface, then risk doesn't just exist; it grows.

That's why I wrote this book: not to criticise, not to point fingers, but to do my part in helping people see some of what I have seen and use it to educate themselves. For me, this is a silent dedication to someone who should have lived longer, and if I share what I've learnt - sometimes the hard way - someone else might see what I couldn't, sooner. It is my hope that nobody else has to stand where I stood that fateful day, because once you've seen it - really seen it - you don't get to look away, and you don't get to go back to seeing safety the way you did before.

As safety professionals, we can't afford to treat risk as abstract. Every minor hazard ignored, every control poorly applied and every unsafe shortcut accepted adds another link to the chain. And when enough weak links go unchallenged, risk stops maturing quietly and starts collecting loudly. The quietest hazards are often the deadliest, and it is our job to shine a light where others assume everything is fine.

If this book helps even one person stop long enough to truly see the risk before it becomes a tragedy, then every page was worth writing.
